20/20 OD and 20/40 OS

EXAM PROTOCOL

The refraction performed revealed hyperopia, astigmatism, and presbyopia OU with BCVA of 20/15 OD and 20/20 OS. Upon ocular examination, various hemorrhages (dot, blot, flame) were found in the retina of both eyes, and hard exudates were found in the retina OD. Pingueculae were present OU and the lenses were starting to exhibit nuclear sclerosis. Her visual fields also revealed some abnormal central defects OU. IOPs were elevated at approximately 30-39 OU.

Diabetic Retinopathy Artificial Intelligence Findings: The images were analyzed by Diabetic Retinopathy Artificial Intelligence Software. The images were reviewed and interpreted by 20/20NOW Doctors. Microaneurisms and exudates were detected.

CONCLUSION

Due to the ocular hypertension, visual field defects, and diabetic retinopathy found in both eyes, the patient was referred for a comprehensive dilated evaluation by an ophthalmologist.

Significant for depression/anxiety and NIDDM diagnosed 20 years ago, with blood sugar levels measured at 140 two days ago "normal" HgA1c measured a month ago. Her reported medications include Metformin and Naproxen.

Hyperopia OU, Astigmatsm OD, and presbyopia OU with best corrected Vas of 20/20 at distance and near OU.

EXAM PROTOCOL

Upon retinal examination, cup to disc ratio was approximately 0.7 OU, and both eyes exhibited macular drusen and mild retinal vessel engorgement. Exudates and hemorrhages were found OS. Anterior segment was significant for narrow angles OU, arcus OU, and possible cataracts OU. The visual field screener exhibited some mild central defects. IOP was measured at an average of 17 mmHg OD and 15 mmHg OS. Diabetic Retinopathy Artificial Intelligence Findings: The images were analyzed by Diabetic Retinopathy Artificial Intelligence Software. The images were reviewed and interpreted by 20/20NOW Doctors. Exudates and microaneurisms were detected in the both eyes.

CONCLUSION

The patient was referred for a comprehensive dilated evaluation for diabetic retinopathy OS and glaucoma suspect evaluation.

Anterior segment evaluation was significant for narrow angles and cataracts OU. Visual fields and IOP were wnl OU. Diabetic Retinopathy Artificial Intelligence Findings: The images were analyzed by Diabetic Retinopathy Artificial Intelligence Software. The images were reviewed and interpreted by 20/20NOW Doctors. Microaneurisms were detected in the right eye.

CONCLUSION

The patient was diagnosed with diabetic retinopathy OU and referred for comprehensive dilated evaluation and monitoring.
